Firearms Packet for Existing ID Card Holder

The following packet is to be used if you are, or have previously been the owner of a Firearms Identification Card and are applying for any of the following reasons:

lost or stolen identification card

mutilated identification card

change of address

change of sex

change of name

application to purchase a handgun

All forms must be signed in person at police headquarters and witnessed by police personnel!

The State Police requires a criminal history background check for persons who already hold a firearms identification card and are applying for any of the above listed reasons.

You must submit the following items:

· $18.00 certified check or money order made payable to the New Jersey State Police S.B.I.

· Form STS-033 (State of New Jersey Application Form) indicating number of permits requested

· Form SP-66 (Consent for Mental Health Records Search)

· Form 212A (Request for Criminal History Record Information) – Signature Only, form to be completed by PD

· Copy of Drivers License

· Copy of Firearms Identification Card - unless lost, stolen or mutilated

If applying for a purchase permit you must also submit:

· $2.00 (per permit) cash or check made payable to the Borough of Upper Saddle River

When your paperwork is ready, the applicant must appear in person, as they are required to sign the permit prior to receiving same.

The police department will contact you when your permit/identification card is ready.

Firearms Packet for New Applicant

The following packet is for anyone who has never applied for a firearms identification card or purchase permit.

All forms must be signed in person at police headquarters and witnessed by police personnel!

FIREARM IDENTIFICATION CARD

A firearm identification card is required by New Jersey Statute for the purchase of rifles and shotguns and may be used as identification to purchase ammunition. To apply for a firearm identification card the applicant will need a certified check or money order in the amount of $60.25 made payable to New Jersey State Police S.B.I. and cash or a check made payable to the Borough of Upper Saddle River in the amount of $5.00. The applicant must also be fingerprinted prior to submitting any paperwork. Fingerprinting is done by appointment only, so please call police headquarters to set up an appointment prior to coming in. You may also apply for a handgun purchase permit at this time for a fee of $2.00 per permit.

You must submit the following items for a firearm identification card:

· $60.25 certified check or money order made payable to the New Jersey State Police S.B.I.

· $5.00 cash or check made payable to the Borough of Upper Saddle River

· Completed applicant fingerprint cards (Set of 3)

· Form STS-033 (State of New Jersey Application Form)

Wait time for firearm identification cards range between 30 days and six months!

When your paperwork is ready, the applicant must appear in person, as they are required to sign the identification card/permit prior to receiving same.

The police department will contact you when your permit/identification card is ready.
